Add support for defining default database when creating connection

When creating MSSQL/Azure SQL databases, you have the ability to create users/logins which authenticates connections at a database level. In order to this to work, you have to define the default database when setting up the connection so the server know at which database the user should be authenticated.

Therefore i would like a optional field for database in the Connection Preferences.
